Using advanced diagnostics, expert-led care and individualized treatments we go beyond symptom relief to target the root causes of nagging symptoms and chronic conditions. Whether supporting gut health, fertility, thyroid and hormonal balance, autoimmune conditions, musculoskeletal pain, headaches, heart health, stress and mood disorders, weight management and a range of unresolved or undiagnosed conditions, Aligned Modern Health treats the whole person with strong doctor-patient relationships and proven clinical outcomes empowering lasting well-being and long-term health.

We are seeking outstanding Nurse Practitioners to join our Functional Medicine practice. We are looking for licensed Nurse Practitioners with experience practicing Functional Medicine and conducting functional diagnostic testing who want to be part of a dynamic organization committed to improving the healthcare landscape.

Our preference is to hire full-time providers but would consider part-time provider options on an individual basis (working additionally in a private practice doing similar work would not be permitted). Active IL APRN license would be required to start. Providers based in FL, MI, TN, and TX, are highly preferred.

The heart of who we are and how we operate is patient first. For full-time providers, we have a starting base salary guarantee of $130K (depending on experience), as well as bonus/revenue share opportunity. We also have a robust benefits package including medical/dental/vision, 401-k, med mal, paid time off, holiday pay, and more! We manage all marketing, staffing, practice management, billing and reimbursement, as well as provide all necessary equipment.

Many of our current functional medicine providers have done extended work/training in the following: American Academy of Anti-Aging Medicine (A4M), American College of Lifestyle Medicine (ACLM), Institute of Functional Medicine (IFM), Diplomate of the American Clinical Board of Nutrition (DACBN), Functional Medicine University (FMU), and the College of Integrative Medicine (CIM) and take a data-driven approach to patient care.
Job Responsibilities:

  • Conduct thorough patient assessments, including medical history, physical examinations, and diagnostic tests
  • Develop personalized treatment plans based on patients' needs and medical conditions
  • Prescribe nutraceuticals and/or medications and monitor their effectiveness
  • Collaborate with other healthcare professionals to coordinate patient care
  • Provide counseling and education to patients on disease prevention and healthy lifestyle choices
  • Maintain accurate and up-to-date patient records using electronic health record systems
  • Stay current with medical advancements, research, and clinical trials in the fields of functional, integrative and lifestyle medicine


Requirements:

  • At least two years of experience practicing functional medicine and performing functional medicine specialty testing/labs
  • Experience in functional and integrative medicine settings preferred
  • NP/APRN with a concentration in Functional Medicine, Hormone Health, Anti-Aging or Lifestyle Medicine
  • Providers based in FL, MI, TN, and TX are preferred
  • Must have (or be able to obtain) an IL APRN license and any remote location determined
  • Strong oral and written communication skills with the ability to collaborate and interact effectively with patients and other providers
  • Energetic and outgoing personality with a desire to help people
  • Patient first attitude and ability to consistently provide a 5-star level of service
  • Knowledge of HIPAA regulations and ability to maintain patient confidentiality
