Overview:

Sentara Quality Care Network (SQCN) is hiring a Social Worker/MSW - Part- time - Flexible Dayshift in Harrisonburg, Rockingham, Mt. Jackson, Broadway, Staunton, and Elkton, VA!

Status: Part-time, permanent position (20 hours)

Work hours: Flexible day shift (Monday-Friday). Can work anytime between 7:30 AM and 6:00 PM EST, as long as they complete the required 20 hours per week.

Location: Remote within the following areas: Harrisonburg, Rockingham, Mt. Jackson, Broadway, Staunton, and Elkton, VA. Candidates must reside within a 30-minute driving distance of these locations. Travel may be required based on patient needs or assigned cases.

Overview

  • As a key member of the Population Health Team, the Social Worker, MSW, will contribute to results as defined by the principles of value-based care.

  • The position will collaborate with, and serve as an expert consultant to the Population Health RNs, providers, and practice staff to engage in management approaches, which optimize desired health outcomes.

  • Responsibilities will include acting as an advocate for the patient (beneficiary, member), family and/or significant others, acting as a liaison between providers, patients, care team members, skilled nursing facilities, home health, hospital staff and community partners/resources.

  • Acts as a resource to assist patients in need of community-based program assistance and empowers the patient with the skills to navigate the process for obtaining appropriate medical care and community assistance.

Education

  • Master's degree in Social Work REQUIRED

Certification/Licensure

  • None required

Experience

  • 2 years' experience in direct patient care and case management REQUIRED

  • Experience working with the frail or elderly population preferred

  • Knowledge of Medicare, Medicaid and CMS regulations preferred.
