The Team Coordinator is primarily responsible for coordinating the day to day workflow of the Home Health team driven through HCHB tasks and direction from the clinical supervisor. This will include successful management of scheduling, quality & compliance, the Lifespark Experience, and financial performance related to productivity and clinical expense.

Position: Team/Scheduling Coordinator for Home Health

Hourly Wage Range: $22-$24/hourly, depending on experience 

Service Area: Hybrid (this position will work between remote and in-office at our corporate office in St. Louis Park 55416)

Schedule: Full-Time 1.0 FTE / Monday – Friday, 8:00am-5:00pm (Rotation. Weekend: 1x per month, Holiday: 1-2/year and no on-call)

Position Functions & Responsibilities: 

  • Scheduling – clients and employees.
  • Scheduling Home Health Aide visits with clinicians and clients.
  • Able to answer phones with appropriate customer service and helps resolve issues or concerns of caller.
  • Coordination with clinicians and billing manager to assure accurate & timely authorizations, orders or paperwork is completed for billing.
  • Quality/Compliance – ensuring visits and documentation completed per organization policy.
  • Seek input from team for improved processes, creating a team environment, and overcoming any obstacles.
  • Seek and share opportunities where technology can improve business process and overall efficiencies.
  • Ensure policies, procedures, and best practices are implemented.
  • Accepts unique client symptoms and behaviors.
  • Front line for all incoming phone calls for team.
  • Data entry for referrals, spreadsheet tracking, medication sheets or other materials needed for staff.
  • Administrative point of contact for Clinical Supervisor to meet needs of team.
  • Follow-up Required – Escalating Event.
  • Provide team coverage to other departments as needed.
  • Other duties as assigned

Qualifications: 

  • At least three years of experience in Home Health/Community scheduling.
  • Home Health and/or Hospice experience preferred.
  • At least 18 years of age and is a high school graduate or equivalent.
  • HCHB (Home Care Home Based) experience preferred.
  • Billing/Customer Services experience preferred.
  • Associate’s degree preferred.
  • Candidate with HUC (Health Unit Coordinator) experience is a plus!
