Job description

We are currently seeking a part-time Monitor for our Wakefield Life Choices Program. In this job, you will work directly with a person supported within the day program during their van rides as a monitor.

SHIFTS
Monday – Friday, Split-shifts from 6:30 am-8:30 am and 2:30-4:30 pm.

ESSENTIAL DUTIES AND RESPONSIBILITIES

  • Provides assistance to a person supported consistent with both their physical needs and defined support plan. Ability to adapt and make adjustments to techniques based on functional abilities and limitations. This includes complete familiarity with the profile and history of the individual support and active indications that care and assistance follows the prescribed needs.
  • Must have knowledge of and the ability to follow the Individual Support Plan.
  • Follows clinical and behavioral treatment plans, record target behaviors on data sheets and submits daily reports.
  • Ensures the safe and punctual transportation of individuals to their vocational program/place of employment and to and from home.
  • Provides assistance as needed to people supported when entering and leaving the vehicle.
  • High school diploma or GED required, further education preferred.
  • Direct support experience managing behaviors strongly preferred.
  • Experience working with families of individuals and professional service providers is preferred.
  • Valid MA Driver’s license required
  • Acceptable CORI report and fingerprint required.
