Licensed Practical Nurse (LPN) - Part-Time

One Home Health | Northwest Indiana

About the Role

One Home Health is growing, and we're looking for a compassionate and skilled Licensed Practical Nurse (LPN) to join our team on a part-time basis. This position provides in-home nursing care to patients throughout Northwest Indiana, with opportunities available in:

  • Lake County: Crown Point, Hobart, Gary, Hammond, Dyer, and surrounding communities
  • Porter County: Valparaiso, Portage, Porter, Hebron, and surrounding communities

What You'll Do

As a Part-Time LPN, you will:

  • Provide professional nursing care under the supervision of a Registered Nurse and in compliance with the Indiana Nurse Practice Act
  • Observe, assess, and promptly communicate changes in patient condition according to agency guidelines
  • Promote a safe and supportive environment for patients, families, and team members
  • Follow established agency policies, procedures, and professional standards
  • Assist with the coordination and delivery of home health services
  • Participate in required meetings and maintain accurate, timely documentation
  • Utilize glucometers and INR monitoring devices as needed
  • Perform basic wound care according to physician orders and individualized care plans
  • Complete other duties and responsibilities as assigned
What You'll BringRequired Qualifications
  • Graduate of an accredited nursing program
  • Current and active Indiana Licensed Practical Nurse (LPN) license
  • Minimum of one (1) year of acute care nursing experience or equivalent clinical experience
  • Current CPR certification
  • Valid driver's license and reliable, insured transportation that meets agency requirements
  • Clean driving record
