About Us

North Valley Specialty Group, Inc. is a California-based mobile wound care clinic specializing in advanced wound management for medically complex patients in home and facility settings. Our clinicians deliver evidence-based care — including cellular and tissue-based products (CTPs/skin substitutes), sharp debridement, and comprehensive wound assessment — directly to patients who cannot access traditional outpatient settings. We are Medicare-enrolled and operate under the supervision of our Direct of Clinical Operations and Medical Director.
Position Overview

We are seeking an experienced, driven Nurse Practitioner (FNP-C/FNP-BC) or Physician Assistant (PA-C) to join our growing mobile wound care team. The ideal candidate has hands-on experience with advanced wound care protocols, including the application and documentation of skin substitute grafts (Q-codes), and is comfortable working independently in field-based environments.
Key Responsibilities

  • Perform comprehensive wound assessments and develop individualized plan-of-care for patients with acute and chronic wounds
  • Apply and document cellular and tissue-based products (skin substitutes) in compliance with LCD L38780 and applicable Medicare coverage criteria
  • Perform sharp/selective debridement (CPT 97597, 97598, 11042–11047) and other advanced wound care procedures
  • Document clinical encounters accurately and thoroughly in the EHR (Intellicure), supporting audit-defensible charting standards
  • Order and interpret relevant diagnostic studies (ABI, A1c, albumin, CBC) to support wound care planning and Medicare compliance
  • Coordinate with the supervising/collaborating physician, nursing staff, and billing team to ensure continuity and compliance
  • Maintain wound care logs, product tracking (lot numbers, wastage), and Q-code documentation per CMS and Noridian MAC standards
  • Participate in quality assurance reviews and ongoing compliance education
Qualifications

  • Active California NP or PA-C license in good standing (required)
  • Medicare-eligible provider with active NPI (required)
  • Minimum 1–2 years of PA/NP experience 
  • Wound care experience, preferably in a mobile, home health, or SNF setting is a plus but not required
  • Demonstrated experience with CTP/skin substitute application and Q-code billing documentation
  • Familiarity with LCD L38780 coverage criteria strongly preferred
  • Comfortable with independent field work and self-directed scheduling
  • Strong written documentation skills; EHR experience required (Intellicure experience a plus)
  • Valid California driver's license and reliable transportation required

Compensation / Schedule

NVSG offers a straightforward, transparent pay structure designed to reward clinical productivity. Providers are compensated at $125.00 per treatment patient visit, with no administrative overhead or billing burden on your end. Per diem and part-time arrangements are available and welcomed — ideal for experienced wound care clinicians seeking schedule flexibility without sacrificing the quality and clinical depth of a specialty practice. Malpractice coverage is provided for all patient encounters.
