Job description

Responsible for planning, coordinating, and documenting care for an assigned caseload of patients. Utilizing the physician's orders and professional skills, the Home Health RN, in conjunction with the Case Manager, develops and implements a home health plan that meets each patient's specific needs and is in compliance with accrediting organization standards, Federal and State regulations, reimbursement guidelines, as well as agency policies and procedures. Participates in Quality Assessment and Performance Improvement (QAPI) activities.
Duties:

  • Making the initial evaluation visit to Oasis Start of Care (SOC)
  • Regularly re-evaluating the patientโ€™s nursing needs
  • Initiating the plan of care and necessary revisions
  • Furnishing those services requiring substantial and specialized nursing skill
  • Initiating appropriate preventive and rehabilitative nursing procedures
  • Preparing clinical and progress notes, coordinating services, and informing the physician and other personnel of changes in the patientโ€™s condition and needs
  • Counseling the patient and family in meeting nursing and related needs
  • Participating in in-service programs, supervising, and teaching other nursing personnel
Regulatory Requirements:

  • Licensed Registered Nurse in California
  • Credentials in specialty areas, i.e., phlebotomy, IV, ET.
  • Current BCLS, CPR card.
  • Wound care experience preferred.
  • One (1) year of acute care nursing or Skilled Nursing experience.
  • Prior experience in home health is preferred, but we can train if a new grad (NEW GRAD IS WELCOME TO APPLY)
  • Valid California Driverโ€™s License, current automobile insurance, and reliable automobile.
Language Skills:

  • Able to communicate effectively in English, both verbally and in writing.
  • Additional languages preferred.
Skills:

  • Computer Knowledge to work on Electronic Medical Records (EMR)
  • Knowledgeable of current Federal, State, and Agency regulations on patient home healthcare.
  • Strong clinical assessment, communication, organization, and documentation skills.
