As a Home Health Registered Nurse, here's a big-picture view of what you'll do:

  • Provide one-on-one, home-based care for an adult patient, including tracheostomy (trach) and gastrostomy tube (G-tube) management, and medication administration
  • Monitor vital signs, assess patient conditions, and document findings accurately in patient records
  • Maintain open communication with the RN supervisor, care team, and family members to ensure continuity of care
  • Carry out the patient's plan of care, including therapeutic interventions and rehabilitation exercises as prescribed
