Your Role as a RN:

We are seeking aRegistered Nurse (RN) Case Managerto provide skilled nursing care to our home health patients. This position is available as
part-time, with flexible scheduling. Care locations would be inDavis & Weber Counties.

You'll work closely with an interdisciplinary team to:

  • Conductadmission and OASIS assessments
  • Performhead-to-toe and wound evaluations
  • Create and implement individualizedplans of care
  • Providefollow-up skilled nursing visits
  • Educate patients and caregivers
  • Coordinate care with MDs, CNAs, PT/OT, and family members
  • Attendbi-weekly team meetingsand contribute to care planning
