Job description

The Admissions Department on the Manhattan campus is seeking an  RN Admissions Evaluator,  who will work on weekends, Saturday and Sunday from 9 am to 5 pm,  to join our team.  This is a remote assignment.

Centralized Weekend Admissions' Goal:

  • To have the ability to streamline all weekend referrals through one centralized location based upon bed availability and appropriateness of referral

Job requirements/ responsibilities:

  • 1 RN will work Sat/Sun (9am to 5pm) to field and respond to all referrals for the two campuses via Careport and Navihealth onsite
  • RN will be cross- trained on the admission process for each division’s criteria:
    • Includes ability to check insurance via DORADO (MCA) and e-pace (MAP) and verification of supplemental insurance policies
    • Obtain authorizations from HMO’s
    • Interface with pharmacy either in person or via on call to verify all current medications are in stock  (SRN-does not have pharmacist on site on weekends, MH has a pharmacist onsite on Saturdays)
    • Reconcile Census for both campuses-Confirming admissions from previous day and hospitalizations
    • Confirm vaccination status in CIR Registry
    • 1-800 number for centralized intake or all admissions to be forwarded to onsite RN
  • When  a referral is identified and accepted for admission the following will occur:
    • Verification of insurance benefits
    • Medication verification with pharmacy
    • Admissions application with designated representative or family
    • Input data into My Unity

Job Qualifications:

1. Graduate of BSN, OT, PT or Social Work, with NY professional license in their respective fields.

2. MDS experience preferred.

2. Previous RN experience in a health care setting preferred.

4. PRI certified preferred.

5. Previous experience in post-acute admissions processes required.

5. Acute Care experience preferred.

6. Bilingual a plus.

SPECIALIZED SKILLS AND COMPETENCIES:

1. Responds politely and helpfully to telephone and in-person requests for service consultations.

2. Excellent writing and clinical assessment skills.

3. Good working relationships with staff and referral services.

4. Ability to multi-task and work accurately in a fast paced environment.

5. Knowledge of Electronic Medical Record, Microsoft outlook, Word, and Excel.

6. Required to speak and write in an understandable manner.

7. Bilingual (English/Spanish) a plus.

OTHER SKILLS AND COMPETENCIES:

1. Ability to relate to adult and geriatric populations in a manner that respects their needs and capabilities.

2.  Thorough knowledge and understanding of medical terminology, conditions and treatments relevant to adult and geriatric populations.

3. Ability to make thorough and accurate bio-psychosocial assessments of adult and geriatric applicants in relation to the continuum of services provide by the facility.
