The RN will provide care in the following areas:

  • Skokie
  • Rogers Park
  • Edgewater
  • Lincoln Square
  • Lake View
  • Lincoln Park
  • Downtown Chicago
  • Surrounding areas

Responsibilities

  • Completing the nursing process in the delivery of patient care, including assessment, planning, implementation, and evaluation of patient outcomes
  • Total oversight of patients from admission to discharge
  • Developing a plan of care based on findings
  • Serving as the patient's advocate and maintaining ongoing communication with physicians, referral sources, caregivers, and facility management to coordinate care
  • Timely completion of all skilled nursing documentation
  • Providing direct supervision of nursing clinical care team members (LPN/LVNs & HHAs)
  • Working within a multidisciplinary team approach, coordinating with PT, OT, ST, MSW, HHA's, and office staff

Requirements

  • Current Illinois State RN License
  • CPR Certification
  • Previous experience in nursing or other healthcare field experience
  • Familiarity with medical software and equipment

  • Ability to thrive in a fast-paced environment
  • WellSky EMR knowledge is preferred but not required
