Job description
Provides direct and indirect patient care under the direction of an RN or physician. Assists patients with activities of daily living, provides for personal care, emotional support, and performs more complex clinical skills under the direction of an RN.
Duties and Responsibilities:
- Performs patient care responsibilities considering needs specific to the standard of care for the patient's age.
- Provides patient care that meets the psychosocial and physical needs of the patient within the CNA's scope of practice.
- Assists with patient admissions, discharge, and transfers.
- Provides morning care which may include bed bath, shower, whirlpool, oral hygiene, combing hair, back care, dressing patients, changing bed linen, cleaning overbed table and bedside stand, straightening room, and other general care as necessary throughout the shift.
- Provides evening care which includes hands/face washing as needed, oral hygiene, back rubs, peri-care, freshening linen, cleaning over-bed tables, straightening room, and other general care as needed.
- Provides general nursing care such as positioning patients, lifting and turning patients, applying/utilizing special equipment, assisting in the use of bedpan, commode, and ambulating patients.
- Prepares patients for meals; serves and removes food trays, and assists with meals or feeds patients if necessary.
- Report out of range VS to the nurse promptly. Obtains repeats VS if necessary and performs a manual BP as needed.
- Answers patients' call lights, anticipates patients' needs, and makes rounds of assigned patients.
- Document care in the interactive view thoroughly and promptly.
- Demonstrates the ability to perform procedures within the CNA scope of practice per state law.
- Records a prompt and accurate intake and output every 4 hours.
- Report patient needs and complaints to the patient's RN on time.
- Observes patient's mental and physical conditions, reports any changes to the supervising RN.
- Assists the supervising RN in arranging transfer of patients to another medical facility: print transfer paperwork, call the transfer center, etc.
- Follows infection control procedures.
- Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/health care errors.
- Knowledgeable and knows responsibilities in all code procedures: code red, code green, code pink, code grey, code black, and code red.
- Treats all patients with dignity and respect. Demonstrate compassion and patience with every patient every time.
- Ensures that appearance and personal conduct are always professional.
- Excellent attendance record.
- Wears appropriate clothing for job functions. Wears ID badge.
- Works at maintaining a good rapport and a cooperative working relationship with physicians, administration, and staff.
- Represents the organization positively and professionally in the community.
- Always maintain patient confidentiality.
- Complies with all organizational policies regarding ethical business practices.
- Arrives at work on time and ready to start the report. Manages time wisely throughout the shift and is prepared to leave work when the report is complete.
- Demonstrates the Healing Model.
- Is aware of departmental changes communicated by the manager, via staff meeting minutes, email, or in person. Demonstrates initiative by following through with what is asked promptly.
- Attends 80% of staff meetings and 100% of mandatory competency training each fiscal year.
- Completed all mandatory EBSCO modules.
- Treats co-workers with respect and kindness. Avoid gossiping with co-workers and work as a team player.
- Completely assigned extra duty for the year. Please write a comment about what your duty was and whether you succeeded or failed to accomplish this task. If you do not have a task, please come prepared to discuss ideas for your task this next fiscal year. Examples of extra duties are infection control prevention, chart review, department organization and upkeep, supplies, crash cart, healing model leader, safety champion, birthday board, etc.
Regulatory Requirements:
- Current CNA certification.
