Job description

We are seeking several compassionate and experienced Licensed Clinical Social Worker (LCSW) to provide comprehensive mental and substance use health services within a multidisciplinary healthcare setting for all of CHP sites. The ideal candidate will deliver evidence-based psychotherapy, conduct clinical assessments, and develop individualized treatment plans to support patients across diverse populations, including children, adults, and geriatrics. This role offers an opportunity to make a meaningful impact through patient-centered care, crisis intervention, and case management, utilizing advanced clinical skills in behavioral health and social work.

Duties

  • Conduct thorough patient assessments, including diagnostic evaluations using ICD-10 and ICD coding systems, to inform treatment planning.
  • Provide individual, group, and family therapy sessions utilizing modalities such as cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, applied behavior analysis (ABA), and crisis intervention techniques.
  • Manage cases involving patients with developmental disabilities, autism spectrum disorder, PTSD, Alzheimer's disease, and other chronic or acute mental health conditions.
  • Collaborate with healthcare teams to coordinate patient care plans, including medication management in conjunction with psychopharmacology providers.
  • Maintain accurate medical documentation adhering to HIPAA regulations and utilize Electronic Medical Records (EMR) systems for data collection and record keeping.
  • Engage in clinical research activities related to behavioral health and mental health counseling to support continuous quality improvement.
