Position Overview

Telos Health Systems is seeking a compassionate and motivated Attending Physician (MD/DO) to provide comprehensive on-site primary care services to residents in skilled nursing facilities.

Clinical Responsibilities
  • Provide comprehensive primary care services for residents in skilled nursing facilities
  • Evaluate, diagnose, and manage acute and chronic medical conditions
  • Perform admission evaluations, routine follow-up visits, and medically necessary assessments
  • Evaluate and manage changes in patient condition
  • Collaborate with nursing staff, rehabilitation therapists, specialists, and facility leadership
  • Supervise and collaborate with Nurse Practitioners and Physician Assistants as appropriate
  • Participate in interdisciplinary care planning and quality improvement initiatives
  • Complete timely and accurate documentation within the company EHR
