About the Role

We're looking for an experienced, autonomous Physicianto join our clinical team Part Time at least 3 days per week, providing primary and episodic care toresidents in skilled nursing facilities. This is an independent role builtaround continuity you'll manage your own panel and build long-termrelationships with patients and their families.

Your Day-to-Day

You'll see roughly 16–25 patients a day a mix of long-termchronic care and acute issues. That means annual wellness visits and H&Ps,managing chronic and co-morbid conditions, responding to acute changes,reviewing meds and orders, and coordinating with collaborating specialists andfacility staff to keep care seamless. You'll also stay in touch with familiesand join care conferences where it makes sense.

Qualifications

  • MD or DO from an accredited medical school.
  • Board Certified (BC) required Internal Medicine, Family Medicine, or Geriatrics (or equivalent).
  • Valid, unrestricted physician license in the state of operation.
  • Must be opted-in with Medicare
  • Successful completion of an accredited residency program.
  • Skilled nursing or long-term care experience strongly preferred.
  • Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
