Position Summary

The Wound Care Nurse provides skilled nursing services to patients in their homes, specializing in the assessment, treatment, monitoring, and management of acute and chronic wounds. Working under physician orders and the direction of the Director of Nursing, the nurse develops individualized care plans designed to promote healing, prevent complications, and improve patient outcomes.

This position requires strong clinical judgment, exceptional assessment skills, and a passion for delivering compassionate, patient-centered care.

Responsibilities

  • Perform comprehensive nursing assessments.
  • Assess, stage, and monitor pressure injuries and chronic wounds.
  • Perform wound measurements and documentation.
  • Provide wound cleansing, debridement (within scope of practice), dressing changes, packing, and negative pressure wound therapy (Wound VAC), if trained.
  • Administer medications and treatments as ordered.
  • Monitor patients for signs of infection or complications.
  • Educate patients and caregivers on wound care, nutrition, pressure relief, and infection prevention.
  • Collaborate with physicians, nurse practitioners, therapists, and other healthcare professionals.
  • Obtain physician orders when changes in treatment are indicated.
  • Complete timely and accurate electronic documentation.
  • Participate in interdisciplinary case conferences.
  • Maintain compliance with Medicare, Medicaid, HIPAA, and agency policies.

Qualifications

Required

  • Current unrestricted Virginia RN or LPN License
  • Current CPR/BLS Certification
  • Valid Driver's License
  • Reliable transportation
  • Current automobile insurance
  • Ability to travel throughout assigned service areas
  • Excellent communication and assessment skills
  • Ability to work independently
  • Basic computer and EMR proficiency

Preferred

  • Home Health experience
  • Wound care certification (WCC, WOCN, CWCN, or equivalent)
  • Experience with wound VAC therapy
  • Experience treating diabetic ulcers, venous ulcers, arterial ulcers, pressure injuries, and surgical wounds
  • Experience with Medicare, Medicaid, and OASIS documentation
  • Experience using Alora or similar EMR systems
