Job description

Community Health Partnership of Illinois (FQHC) is a 501 c 3 nonprofit organization seeking drivers for Champaign, Kankakee, and Princeville surrounding areas. Duties include, but are not limited to; delivering packages from site to site, transportation patients to and from home to medical appointment, support medical outreach and education in a timely manner and providing front office support as needed. The driver is responsible for operating the CHP Mobile Clinic by arriving early to outreach events getting the Mobile clinic ready for patient care. Drivers may be required to work nights and weekends to support outreach activities. The ideal candidate should have a clean driving record, be detailed oriented and safety conscious.

Common duties and requirements are:

· Must have a CDL license.

· Transport clients and packages to and from Health Center Clinics.

· Transport clients/patient to and from medical appointments.

· Arrive at destinations on schedule.

· Fulfill administrative needs, like office pickups.

· Research and plan for traffic, construction and weather delays.

· Use navigation applications to determine the best route.

· Maintain a route log daily, mileage records and repair records up to date.

· Always interact with clients professionally.

· Ensure that the vehicle is always clean, meets state requirements, fueled and ready for use.

· Arrange for vehicle repairs.
