Job description

We are seeking a 3-11 Full-Time LPN in our Assisted Living Neighborhood
Responsible for performing duties such as, but not limited to, those outlined below. Renders nursing care to residents within the assigned nursing unit in support of medical care, pursuant to the objectives and policies of this facility. Exercises good judgment and respects confidential matters. Works effectively with others to ensure that quality of care and quality of life for each resident are provided in compliance with Federal, State and local requirements. Responsible to oversee CHHA's and CNA's according to New Jersey Nursing Standards for the LPN and as designated by the Wellness Director.

Responsibilities
  • Provide nursing care to residents in accordance with physician orders, accepted clinical practices, and facility policies and procedures.
  • Administer medications and treatments safely and accurately, including proper storage and accountability for controlled substances.
  • Perform nursing services such as vital signs, treatments, medication administration, and assistance with Activities of Daily Living (ADLs) as needed.
  • Observe, assess, and evaluate residents upon admission and daily; promptly report changes in condition to the Wellness Director, RN, physician, or appropriate interdisciplinary team member.
  • Participate in the development, implementation, and updating of individualized care plans.
  • Collect and communicate resident data for assessments, including participation in the care planning process.
  • Document care, treatments, episodic events, incidents, and follow-up interventions in the clinical record according to professional standards.
  • Communicate unusual observations, emergencies, family concerns, and care issues in a timely and professional manner.
  • Assume responsibility for coordinating resident care on the unit, including completing the 24-hour nursing report and providing effective shift-to-shift communication.
  • Receive, verify, and ensure accurate transcription and implementation of physician orders when delegated or in the absence of the Charge Nurse.
  • Supervise and direct aides to ensure care plans are followed and resident needs are met; adjust assignments as necessary.
  • Assist with restorative, rehabilitative, and therapeutic nursing activities as outlined in resident care plans.
  • Promote residents’ rights by maintaining privacy, dignity, confidentiality, and a respectful environment at all times.
  • Provide emotional support to residents by recognizing and respecting their psychosocial, cultural, religious, and personal needs.
  • Collaborate effectively with residents, families, physicians, and interdisciplinary team members to promote quality outcomes.
  • Respond promptly to resident requests and ensure immediate needs are addressed regardless of assignment.
  • Maintain a clean, safe, and organized environment while adhering to infection control and safety protocols.
  • Follow emergency procedures and demonstrate knowledge of fire safety, disaster preparedness, and evacuation plans.
  • Complete incident reports accurately and promptly after ensuring resident safety.
  • Attend required staff meetings, in-services, and continuing education programs to maintain licensure and professional competency in accordance with New Jersey Board of Nursing requirements.
  • Assist with the orientation and training of new nursing staff and serve as a preceptor when designated.
  • Address staff concerns objectively and escalate issues to nursing leadership when appropriate.
  • Demonstrate professionalism, reliability, compassion, and the ability to work collaboratively under supervision.
  • Perform other duties as assigned to support resident care and facility operations.
