General Summary
Facilitates recovery and enhances wellness by providing housing support and care management in order to increase individual's ability to live independently in the community. Assists individual in obtaining and retaining living arrangements of their own choice. Assists individual in becoming socially and physically integrated into the community and reduces the individuals' need for more intensive behavioral health services. Develops and maintains a positive working relationship with community agencies, participates in multidisciplinary treatment team meetings in order to collaboratively support individuals participating in housing support programs.

Duties and Responsibilities
Essential Functions:
  • Actively facilitates treatment planning, utilization management and discharge planning with the individual receiving housing services and the treatment team.
  • Coaches and/or teaches skills related to psychoeducation, understanding diagnosis, triggers and basic coping skills, advocacy, understanding stigma, empowerment and recovery support.
  • Coaches and teaches skills related to Activities of Daily Living, included but not limited to food preparation, meal planning, laundry, personal hygiene, household organization, employment/volunteer activities, organizing personal schedule, budgeting, transportation, socialization and/or other housing goals.
  • Provides assistance with the location of housing, roommate matching, renter skills training and/or landlord/tenant negotiations as outlined by housing program.
  • Completes all required documentation for individual documentation to maintain services (ie-Intake Assessment, Outcome Measures, C-SSRS, Treatment Plan, Safety Plan and other clinical documentation).
  • Acquires knowledge or resources for Lebanon County while maintaining an excellent working relationship with all providers and community agencies.
  • Acts as a liaison with other community agencies/groups, funding sources and businesses that relate to the services provided to individuals in housing programming.
  • Provides support and/or assistance and consultation to families, social supports, landlords, and community agencies of the individual.
  • Assists in providing access to behavioral health services throughout Lebanon County.
  • Supports individual in community setting such as teaching community safety, assist with transportation efforts (ie-public transportation), support individual identified access to spiritual needs, accompany to other socialization activities as it relates to the individual's goal.
  • Serving as an on-going liaison to community resources, through referrals and accompanying the individual to appointments when appropriate.
  • Completes necessary documentation for each individual encounter within 48 hours of provided service.

  • Attends all meetings with supervisor, staff, and team meetings as well as trainings and in-services.
  • Models in all interactions respect, trauma-informed care, and recovery model.
  • Advocates and informs regarding mental health and resiliency.

  • High School Diploma or GED Required

Work Experience:
  • 2 years In related work experience. Required

Driver's License Statement:
Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.

  • Be trauma informed, culturally competent and recovery oriented in practice.
