Providence Life Services offers a full range of services for families who need retirement living, assisted living, rehab, skilled nursing, and more. Providence Hospice is a growing division of Providence Life Services and is currently seeking a Hospice CNA, Part-Time willing to travel to South Holland, and surrounding areas.


This is an excellent career opportunity for individuals who are truly passionate about providing care for the elderly!


The primary purpose of your job position is to provide each of your assigned clients with excellent and individualized personal care and services in accordance with the client's hospice aide plan of care as developed by the nurse case manager. The current schedule is 10- 15 hours/week. Mileage will be reimbursed at $0.76/mile, and the hourly rate is $18.80 to $21.00.


Summary of essential job functions

  • Assists with all ADL's including bathing (Activities of Daily Living), Medication Reminders, and Vitals, etc.
  • Support and represent the mission of Providence as shown by your actions while on duty.
  • Document changes in medical condition to the appropriate primary care nurse.
  • Give and receive shift reports to ensure consistent medical care is delivered.
  • Measure and record vital signs as necessary or instructed.

Requirements

  • Must be a CNA in the State of Illinois.
  • Must have a compassionate, empathetic and nurturing personality.
  • Home health and/or hospice experience strongly preferred.
  • Comfortable traveling throughout the organizational geographic region.
  • CPR certification (preferred)
  • Current Driver's license with good driving record
  • Reliable transportation
