Job description

Summit Home Care and Hospice is searching for an experienced and motivatedย LPNย to join our Home Healthย team.ย At Summit, we believe healthcare should be personal, compassionate, and delivered where it matters mostย -ย at home.ย 

Licensed Practical Nurse (LPN):ย 

  • Hours:ย Mondayย -ย Friday; Standard Business Hoursย & Occasionalย On-Call Rotationย 

  • Work Setting:ย On the Road for Patient Visits & Training at the Local Office or Remotelyย 

  • Territory:ย ~30-Mile Radius (Mileage Reimbursed)ย 

  • Pay Rate:ย $28.00ย -ย $31.00 per Hourย 

  • Location:ย Milford, OH & Surrounding Areas

Responsibilities:ย ย 

  • Provide direct patient care under the supervision of a registered nurse.ย ย ย 

  • Follow the plan of care, provide treatments, and work collaboratively with the members of the team to help meet positive patient care outcomes.ย ย 

  • Implement the plan of careย initiatedย by the registered nurse.ย ย 

  • Provideย accurateย andย timelyย documentation consistent with the plan of care.ย ย 

  • Assess and provide patient and family/caregiver educationย regardingย self-care techniques and information pertinent to diagnosis and plan of care.ย ย 

  • Participate in coordination of home healthย and hospiceย services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinic Manager.ย ย 

  • Use and/or prepare equipment and supplies effectively and efficiently adhere to aseptic techniques.ย ย 

  • Participate in personal and professional growth and development.ย ย 

  • Perform other duties assigned by the registered nurse, including caring for hospice patients as census allows.ย 

  • Participate in weekendย and holiday on-call rotation requirements.ย ย 

Qualifications:ย ย 

  • Must be a graduate from an accredited practical nurse or vocational nursing program.ย ย 

  • Must hold aย valid/activeย license as a Practical Nurse (LPN) or Vocational Nurse (LVN) in Ohio.ย ย ย 

  • 3 years of nursing experience, preferred.ย ย ย 

  • Prior experience in community/home health and/or medical/surgical, preferred.ย ย ย 

  • Comply withย accepted professional standards and practices.ย ย 

  • Demonstrates good verbal and written communication, and organizational skills.ย ย 

  • Must be able to lift, position or transfer patients and lift supplies and equipment.ย ย ย
