Summit Home Care and Hospice is searching for an experienced and motivated Registered Nurse (RN) to join our Hospice team. Weโ€™re committed to providing comprehensive healthcare services in the comfort of our patients' homes to positively impact quality of life by redefining the delivery of care.ย ย 

Full-Time Registered Nurse (RN):ย 

  • Hours: Monday - Friday; Standard Business Hours & Occasional On-Call Rotationย 

  • Annual Salary: $75,000 - $85,000 per Year (Overtime Eligible)ย 

  • Sign on Bonus:ย $5,000 - $10,000 based on Previous Hospice Experienceย ย 

  • Location/Territory: Westerville,ย OH & Surrounding areas OR Grove City, OH & Surrounding areas

  • Work Setting: On the Road for Patient Visits & Training at the Local Office or Remotely

Responsibilities:ย ย 

  • The Registered Nurse (RN) plans, organizes and directs home care services and is experienced in nursing, with an emphasis on community health education/experience.ย ย 

  • The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individuals and families within their homes and communities.ย ย 

Patient Careย 

  • Completes an initial assessment of patient and family to determine home care needs.ย 

  • Provides a complete physical assessment and history of current and previous illness(es).ย 

  • Regularly re-evaluates patient nursing needs.ย 

  • Initiates the plan of care and makes necessary revisions as patient status and needs change.ย 

  • Uses health assessment data to determine nursing diagnosis.ย 

  • Develops a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and rehabilitative nursing actions. Includes the patient and the family in the planning process.ย 

  • Initiates appropriate preventive and rehabilitative nursing procedures.ย ย 

  • Administers medications and treatments as prescribed by the physician.ย ย 

  • Provides direct patient care as defined in the State Nurse Practice Act.ย 

  • Counsel the patient and family in meeting nursing and related needs.ย 

  • Provides health care instructions to the patient as appropriate per the assessment and plan of care.ย 

  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.ย 

  • Acts as Case Manager when assigned by Clinic Manager and assumes responsibility to coordinate patient care for assigned caseload.ย ย 

Communicationย 

  • Prepare clinical notes and update the primary physician when necessary and at least every 60 days.ย 

  • Communicate with the physician regarding the patient's needs and report any changes in the patient's condition; obtain/receive physician's orders as required.ย 

  • Communicate with community health related people to coordinate the care plan.ย ย 

Additional Dutiesย 

  • Participate in on-call duties as defined by the on-call policy.ย 

  • Ensure that arrangements for equipment and other necessary items and services are available.ย 

  • Instruct, supervise, and evaluate home health aide care provided every two (2) weeks.ย ย 

Qualifications:ย 

  • Graduate from an accredited school of nursing, associate or bachelorโ€™s degree.ย 

  • Valid/active nursing license.ย ย 

  • 1 year of previous experience as a RN requiredย 

  • Must be a licensed driver with an automobile in good working order and insured in accordance with the organization's requirements.ย 

  • Valid CPR certification is required.ย ย 

  • Recent TB test, required.ย ย 

  • Prior home health care or acute care experience preferred.ย ย 

  • Management experience preferred but not required.ย ย 

  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.ย 

  • Requires prolonged or considerable walking or standing and considerable reaching, stooping, bending, kneeling or crouching.ย 

  • Must be able to lift, position or transfer patients and lift supplies and equipment.ย ย 

  • Must have visual acuity and hearing to perform required nursing skills.ย
