Overview
Interim HealthCare of Montgomery County is seeking a flexible and experienced Registered Nurse (RN) to support both our Hospice and Home Health services in a dual-role capacity.
This position is ideal for an RN who is confident in clinical assessments, recertifications, and end-of-life care, and is looking for a role that offers variety, autonomy, and meaningful patient impact.
You will play a key role in both:

Key Responsibilities
Hospice Responsibilities
  • Perform hospice admissions and ongoing nursing visits
  • Provide pain and symptom management in accordance with plan of care
  • Support patients and families with education, emotional support, and end-of-life care
  • Participate in IDG (Interdisciplinary Group) meetings
  • Ensure compliance with hospice documentation and regulatory standards
Home Health Responsibilities
  • Complete Start of Care (SOC), Recertifications, and Reassessments
  • Perform comprehensive clinical assessments and OASIS documentation
  • Develop and update plans of care in coordination with physicians
  • Ensure timely and accurate documentation for compliance and billing
  • Collaborate with therapy and support staff to coordinate patient care
Qualifications
  • Active Registered Nurse (RN) license in Tennessee
  • Minimum 1 year of nursing experience (home health or hospice preferred)
  • Experience with OASIS documentation strongly preferred
  • Strong assessment and clinical decision-making skills
  • Ability to work independently in a field-based environment
  • Excellent communication and patient/family education skills
Preferred Experience
  • Hospice admissions and case management
  • Home health SOC / Recert experience
  • EMR experience
Physical Requirements
  • Ability to travel to patient homes within service area
  • Ability to perform patient assessments, including mobility assistance as needed
