Your Role:
    • Deliver personalized physical therapy services to home-based clients.

    • Conduct initial and ongoing assessments to track progress and adjust treatment plans.

    • Perform fall risk assessments at Assisted Living Facilities (ALFs) and community expos.

    • Supervise client care to ensure best practices and quality outcomes.

    • Represent our organization at ALF events and expos, providing educational presentations.

    • Travel to client locations and events as needed.

    • Supervise Home Care clients and caregivers

    • Create and update care plans for non-medical clients

    • Administrative duties including calling PCP offices for orders

What You Need:
  • Licensed Physical Therapist (PT) in the state of Hawaii.

  • 1+ years of experience in a clinical or home-based care setting.

  • Experience with fall risk assessments (preferred).

  • Excellent communication and interpersonal skills to engage clients and the community.

Valid driver’s license & reliable transportation to travel as needed.
