Physical Therapist and Physical Therapist Assistant – Home Health

Pay

PT: $80 per visit

PTA: $55–$60 per visit

Location: O‘ahu, HI (reliable transportation required)
Schedule: Flexible / Per Diem (PRN) – Build Your Own Caseload

We are seeking licensed PTs and PTAs who enjoy building strong patient relationships and delivering personalized therapy services.

Responsibilities
  • Provide skilled in-home physical therapy services
  • Complete evaluations, reassessments, and fall risk assessments (PT)
  • Implement individualized treatment plans to improve mobility, balance, strength, and safety
  • Educate patients and caregivers on home exercise programs and fall prevention
  • Document accurately and communicate patient progress with the interdisciplinary care team
  • Maintain compliance with Medicare home health documentation requirements
Requirements
  • Current Hawaii license as a Physical Therapist (PT) or Physical Therapist Assistant (PTA)
  • Minimum one year of clinical experience (home health experience preferred)
  • Strong documentation, communication, and time-management skills
  • Ability to work independently in the field
  • Valid driver's license and reliable transportation
