Physical Therapist Assistant (PTA) | Part-Time Home Health
Service Area: Northside Suburbs of Chicago, Illinois, including Lake Forest, Glenview, Buffalo Grove, Mundelein, and nearby communities.
Join the One Home Health Team
One Home Health is currently looking for a motivated and caring Physical Therapist Assistant (PTA) to support our home health patients on a part-time basis. In this position, you'll work closely with a dedicated interdisciplinary team while delivering individualized therapy services in patients' homes.
This is an excellent opportunity for a PTA who values autonomy, flexibility, and meaningful one-on-one patient care.
Primary Responsibilities
In this role, you will:
  • Provide prescribed physical therapy treatments and patient education under the direction of the supervising Physical Therapist and according to the physician-approved plan of care.
  • Monitor patient progress and communicate changes in condition, treatment response, or scheduling needs to the appropriate team members.
  • Maintain timely and accurate clinical records that meet agency, state, and federal documentation requirements.
  • Collaborate with nurses, therapists, physicians, and other healthcare professionals to promote positive patient outcomes.
  • Assist with care coordination and participate in case conferences or interdisciplinary team meetings when requested.
  • Follow established infection control practices, safety procedures, and equipment usage guidelines during home visits.
  • Protect patient confidentiality and comply with all HIPAA and privacy standards.
  • Adhere to agency protocols, regulatory requirements, and professional practice standards.
  • Participate in educational programs, staff meetings, and ongoing quality improvement initiatives.
  • Carry out additional duties related to patient care and agency operations as assigned.
Required Qualifications
Candidates should meet the following requirements:
  • Current, unrestricted Physical Therapist Assistant license in the state of Illinois.
  • Graduate of an accredited PTA educational program recognized by the American Physical Therapy Association (APTA).
  • Minimum of six months of PTA experience in a healthcare setting preferred.
  • Strong verbal communication, interpersonal, and clinical documentation skills.
  • Reliable transportation with a valid driver's license and current automobile insurance.
  • Ability and willingness to travel throughout the assigned coverage area to provide in-home patient care.
