General Summary

Provides professional nursing care at an advanced practitioner level in accordance with the principles of patient and family-centered care based on the nursing process and according to the clinical standards of the hospital. Coordinates care planning with other disciplines. Safely and independently administers prescribed treatments, medications, and nursing interventions for more complex groups of patients based on assessment and plan of care. Requires specialized skills necessary to deliver an inpatient level of care independently in a patient's home. Uses Telehealth technology daily.

Qualifications
Minimum Education:

  • Diploma Program Required or
  • Associates Degree Required
  • Bachelors Degree Nursing (BSN) Preferred

Work Experience:

  • 3 years Relevant experience. Required

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required and
  • Basic Life Support Upon Hire Required

Duties and Responsibilities
Essential Functions:

  • Independently completes assessment and collects data on assigned patients according to policy; takes initiative to supplement assessment as more complex issues arise. Develops innovative approaches to solve complex patient care issues. Focuses on important parameters and emerging problems to predict and manage an evolving clinical picture. Establishes contingency plans in anticipation of unexpected events and coordinates emergency action in response to acute risk situations. Demonstrates leadership at the point of care by actively performing caring behaviors with every patient interaction.
  • Demonstrates advanced knowledge and skills to safely and independently administer prescribed treatments, medications, and nursing interventions for more complex groups of patients based on assessment and plan of care. Provides supervision and acts as a resource person for all expected unit skills and equipment. Independently organizes and prioritizes time based on higher acuity patient assignments and increased workload.
  • Independently evaluates and monitors the effectiveness of interventions and progress toward outcomes for more complex groups of patients; analyzes data to make recommendations for changes in the plan of care. Initiates and coordinates interdisciplinary collaboration in evaluation of patient progress. Anticipates and coordinates individualized patient needs for transfer/ discharge and includes patient/family in decisions.
  • Acts as a resource person for other staff with proper documentation.
  • Independently reads current literature. Demonstrates inquiry to improve patient outcomes. Participates with a team to evaluate evidence and determine clinical relevance.
  • Seeks learning opportunities and resources beyond own work environment and incorporates into practice the new knowledge and skills acquired. Required education and competencies completed, in accordance with state requirements. Shares pertinent information and job knowledge, related to readings, educational programs and participation in professional organization activities.
  • Collaboratively works with management to monitor the level of professional practice; assists with the initiation of actions or programs to ensure teamwork and professional practice. Serves as a problem solver to improve outcomes.
  • Mentors/coaches others to improve decision-making and accountability.
  • Promotes cooperation and communication among all team members. Anticipates the potential for conflict and works to quickly defuse any situations involving conflict with patient/families and/or co-workers to promote a healthy work environment. Recognizes multiple factors underlying the conflict situation and chooses and initiates actions that result in the most effective long-term solution. In collaboration with management, implements changes to enhance team communication. Coaches team members in developing insights into their own communication patterns.
  • Variation of duties may result based on the assigned customer care unit(s). This may include differences in the specific clinical skills required of the RN.   • In addition to requirements specified in the license and certification section below, current BLS certification for all areas of nursing practice is required.Current certification may be required for the following areas of nursing practice:Emergency Care Center - ACLS, PALSIntensive Care Center & MSCC Lead - ACLS, PALSL&D - ACLS, NRP, - Experienced L&D nurses will obtain within 6 months and new L&D nurses will obtain at 1 year of experienceNICU - NRPPEDS - PALSPCC/PACU/ENDO - ACLS, PALSProcedural - ACLS
