💼 Job Specifics:
  • Location: 1920 Libal St., Green Bay, WI 54301
  • Must reside within a reasonable distance from Green Bay to allow for patient home visits as necessary.
  • Ability to work from home after training! (Training length varies)
  • FTE Status: 0.1 FTE (8-hours per pay period)
  • Work Schedule: Day Shift, Every Third Weekend (Saturday & Sunday) 8:00 am - 4:30 pm, 1-2 holidays per year.

Join our exceptionally caring home health team and enjoy the balance of working independently while being supported by a dedicated group of colleagues who share your commitment to excellent care. In this role, you'll use your clinical skills to provide skilled, short-term acute care to patients one at a time, giving you the chance to build trusted relationships and make a real difference in their lives. You'll assess patient information such as vital signs, weight, and health-related questions, then use your judgment to determine when follow-up or medical intervention is needed. You'll also connect with providers as needed, offer patient education and coaching, and help patients feel informed, supported, and cared for throughout their journey. By working closely with a multidisciplinary team and community resources, you'll help ensure each patient receives thoughtful, coordinated care. This position also offers day shift hours, mileage reimbursement and/or company vehicle use, and generous continuing education opportunities to support your growth.
Qualifications:
  • Wisconsin RN license is required.
  • Previous RN experience preferred.
  • Valid Wisconsin Driver's License in good standing and liability auto insurance required.
