How you'll contribute as a LPN/LVN
Essential Functions
  • Under the direction of an RN, contributes to the assessment of patients by collecting data for analysis by an RN.
  • Performs plan of care interventions, including specimen collection and wound care. May administer medication.
  • Assists RN with development of plan of care.
  • Assists patients with performing activities of daily living, including personal hygiene, elimination, nutrition, and ambulation.
  • Monitors, documents, and communicates patient's status and reactions to interventions with RN, physician, patient, and families.
  • Documents patient care given.
  • Works as an advocate for the physical and emotional well-being of the patient.
  • Schedules appointments and procedures as ordered by the provider.
  • Obtains insurance re-authorizations as requested for testing and scheduled procedures.

Non-Essential Functions
  • Attends clinic meetings.
  • Maintains a clean and safe working environment.
  • Reports all incidents, hazards, drug interactions, and initiate appropriate action.
  • Completes safety instructional programs.
  • Assists other team members when time permits.

FLSA: Non-exempt
Education: Graduate of an approved practical nursing program
License: Current LPN/PN to practice in the Commonwealth of UT and be in good standing with the Board of health Professions
Certifications: BCLS within 30 days of hire/transfer
Minimum Work Experience:
Knowledge of ICD-9, ICD-10, CPT codes, insurance benefits and healthcare backgrounds Required
