Klarus Home Health is seeking a compassionate and skilled Registered Nurses to join our team PRN / Per diem completing admissions for our new Home Health patients on the weekends in the Fort Worth, TX and surrounding areas.
The primary functions of this role are to perform admissions and administer nursing care for patients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family, and referring agency, and assume the responsibility for coordination of care.
Requirements
  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing.
  • Licensure: Current state license as a Registered Nurse. Proof of current CPR certification. Current Driver's License.
  • Experience: Two years' experience as a Registered Nurse in a clinical setting and one-year full-time home health experience is required
  • Knowledge of OASIS is required
  • Skills: Strong assessment and clinical skills, excellent communication skills, ability to work independently and as part of a team. Familiarity with infection control measures (OSHA). Knowledge of State and Federal regulations. Good interpersonal skills.
  • Home Care Home Base (HCHB) knowledge
  • Works in patients' homes in various conditions and locations.
  • Assess home health patients utilizing the Nursing Assessment and LCD criteria to identify physical, psychosocial, and environmental needs.
  • Ability to work a flexible schedule; ability to travel locally
  • PRN emergency call availability.
  • Develop and document the plan of care in correlation with the assessment.
  • Provide nursing care utilizing infection control measures (OSHA).
  • Work with the Interdisciplinary Group to ensure patient/family needs are met.
  • Communicate with attending physicians, home health physicians, and other physicians involved in the patient's care.
  • Timely submission of all required paperwork.
  • Demonstrate commitment, professional growth, and competency.
  • Meet productivity standards.
  • Perform on-call responsibilities and provide on-call service to patients/families as assigned.
  • Provide effective communication to patient/family, team members, and other healthcare professionals.
  • Transportation: Reliable transportation and valid driver's license. Valid and current auto liability insurance.
