Job Summary:

We are seeking a skilled and compassionate Home Health Visiting Nurse to provide in-home care to patients across Chicagoland Area. The ideal candidate is clinically strong, reliable, and able to work independently while maintaining clear communication with our interdisciplinary team.

Responsibilities:

  • Conduct home visits to assess patient conditions and deliver skilled nursing care

  • Implement and evaluate individualized care plans

  • Provide wound care, medication administration, IV therapy, and chronic disease management

  • Educate patients and caregivers on care instructions, safety, and wellness

  • Document care visits and updates using our electronic health record system

  • Coordinate care with physicians, therapists, and other care providers

  • Comply with all state and federal regulations, including HIPAA and infection control protocols

Requirements:

  • Active RN or LPN license in the state of Illinois.

  • Minimum [1-2] years of clinical nursing experience (home health preferred)

  • Current CPR certification
