Job description

Description of the Role:

We are seeking a compassionate and dedicated Home Health Registered Nurse Case Manager to join our team on a part-time basis. In this role, you will be responsible for telephonic care management and support to patients in Allentown, PA and the surrounding counties. 

Responsibilities:

  • Reviewing patient assessments and developing individualized care plans with the care team
  • Ensuring all skilled nursing care, including wound care, medication management, and IV therapy is provided as ordered
  • Coordinating and communicating with healthcare team members to ensure comprehensive and effective care is provided
  • Monitoring patient progress and adjusting care plans as needed
  • Educating patients and their families on proper care techniques and self-management
  • Documenting Transfer and Discharge Oasis as needed

Requirements:

  • Valid Registered Nurse (RN) license in the state of Pennsylvania
  • Minimum of 1 year of experience in home health or similar setting
  • Minimum of 1 year of experience in a case management role
  • Excellent clinical and critical thinking skills
  • Strong communication and interpersonal skills
  • Ability to work independently and manage time effectively
  • Valid driver's license and reliable transportation
