Employment Status: PART TIME
Working Hours: 20 hours a week (40 Hours per pay period) 3 Days one week, 2 days the following week.
Shift: 1st shift (Every 4th weekend and after hours on call)
Position Reports to: Clinical Supervisor
Department Description: Wilson Health's Home Health amp; Hospice Care is designed to provide comprehensive healthcare to patients in their own home on a temporary basis. The care is provided under the direction of a physician, and the goal is to help patients regain their independence after an illness, injury or hospitalization. The goal for patients with chronic illnesses may be to keep them at home safely and as pain-free as possible. Services provided are Skilled Nursing Care, Home Health Aide Care, Social Worker Care, Physical Therapy Care, Occupational Therapy Care, and Speech Therapy Care.
Job Summary: The Home Health Care amp; Hospice Registered Nurse is responsible for the clinical care of the client in the client's place of residence utilizing the nursing process, following established policies and procedures and correlated and in support of the medical plan for care. Coordinates the case management and documentation progress of the client.
  • This position is based in Sidney and may require occasional travel to healthcare facilities and patient homes, within the following 6 county radius of Auglaize, Champaign, Darke, Logan, Miami and Shelby Counties.
  • The RN Case Manager will work in a fast-paced environment that demands flexibility, critical thinking, and a patient-centered approach.
Essential Duties amp; Responsibilities:
  • Use the nursing process and perform initial holistic nursing assessment of the client.
  • Establish short and long term goals, provide skilled nursing service in the home.
  • Document Oasis accurately and completely.
  • Complete Necessary forms required for payer billing.
  • Assess safety of the patient environment.
  • Develop a comprehensive plan of care based on nursing assessment and oasis.
  • Complete all referral forms to appropriate referrals.
  • Review, teach, and administer medications in a safe and timely manner.
  • Recognize and utilize opportunities for health teaching and provide patient education.
Qualifications:
  • Current and valid Registered Nurse (RN) license.
  • Minimum of 2 years nursing experience, preferably in case management or related field.
  • Strong knowledge of healthcare systems, insurance processes, and patient advocacy.
  • Excellent communication, organizational, and problem-solving skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Proficiency with electronic medical records (EMR) and case management software is a plus.
  • Previous Home Health/Hospice experience preferred, Medical/Surgical and Long Term Care experience helpful
