Licensed Practical Nurse (LPN) – Group Home

Location: Queens, NY
Position Type: Part-Time
Shift: 7:00 PM – 7:00 AM | Tuesday, Friday, Saturday & Sunday

Position Overview

Bond Health is currently hiring a Licensed Practical Nurse (LPN) for a part-time opportunity in Queens, NY. The LPN will support individuals living in an Intermediate Care Facility (ICF) Group Home by promoting health, safety, and wellness.

Key Responsibilities
  • Supervise and assist direct support staff with personal care and daily living activities.

  • Administer medications according to established procedures.

  • Monitor infection control practices and maintain first aid supplies.

  • Document medical findings and report significant changes to the RN Supervisor.

  • Communicate with healthcare providers and family members regarding medical concerns.

  • Provide treatments including wound care, G-tube feedings, blood glucose monitoring, urinary drainage maintenance, and range-of-motion therapy as ordered.

  • Monitor vital signs, weights, and other health indicators.

  • Ensure compliance with federal and state regulations.

  • Attend medical appointments and required training.

  • Perform other duties as assigned.

Qualifications
  • Current unrestricted New York State Licensed Practical Nurse (LPN) license.

  • AHA or equivalent CPR certification.

  • Experience with individuals with intellectual and developmental disabilities preferred but not required.

  • Excellent written and verbal communication skills.

  • Ability to work independently and as part of a team.
