Part-time Hospice RN Case Manager with Athena Hospice of Rhode Island

We are seeking a dedicated and compassionate RN Case Manager to join our team. The RN is responsible for planning, organizing, and directing the hospice care of assigned patients under the direction of the clinical team. This is an on-the-road position with mileage reimbursement, and you will be required to service the entire state of Rhode Island. Flexible schedule!

Responsibilities:

  • RN conducts initial, comprehensive, and ongoing assessments of patients and families to determine hospice needs

  • Provide professional nursing care by utilizing all elements of the nursing process

  • Develop and implement individualized care plans that establish goals and incorporate palliative nursing actions

  • RN supervises hospice aides and provide guidance on patient care

  • Collaborate with patients, families, and healthcare professionals to develop a comprehensive plan of care

  • Provide emotional and spiritual support to patients and families

Requirements:

  • Must possess an active and unencumbered RN license in Rhode Island

  • Must have a nursing degree from an accredited college, university, or school

  • Prior RN experience in hospice is preferred
