Schedule: Part-time (22-25 hours/week) - This role will begin Part-time and progress to FULL-TIME over the next 3-6 mos.

Job Description
We are seeking a dedicated and experienced Registered Nurse to join our team supporting individuals with intel lectual and developmental disabilities. The ideal candidate brings strong clinical skills and hands-on experience with DBHDD standards, HRST assessments, updating HRST, Proxy scoring, IDD Connect, HCBS waiver requirements, LPN oversight, and maintaining current and accurate health care plans.
Key Responsibilities
Complete, update, and maintain HRST assessments in alignment with DBHDD requirements
Ensure timely HRST updates when health status, medications, or risk factors change
Conduct Proxy assessments and ensure accurate scoring and documentation for DSPs
Develop, maintain, and update current and accurate health care plans based on assessed needs
Review health care plans regularly to ensure they reflect the individuals present condition and supports
Navigate and update IDD Connect for service authorizations, documentation, and compliance
Apply knowledge of HCBS waiver standards to ensure service delivery meets regulatory expectations
Provide clinical oversight, guidance, and support to LPNs
Monitor health status, risk indicators, and clinical needs of individuals served
Collaborate with interdisciplinary teams to implement person-centered care plans
Train and support direct support professionals and caregivers on health-related protocols
Participate in audits, quality reviews, and continuous improvement initiatives
Ensure compliance with DBHDD regulations, HCBS requirements, agency policies, and best-practice standards

Qualifications
Active Georgia RN license
Proficiency in updating HRST
Knowledgeable in Proxy assessments, IDD Connect, HCBS requirements, and maintaining accurate health care plans
Prior experience providing oversight to LPNs (preferred)
Strong assessment, documentation, and critical-thinking skills
Ability to work independently and collaboratively
Commitment to person-centered, high-quality care
Ability to pass a background check
Experience with Therapy and IDD Connect a plus
Experience
3-5 years of experience working with people with mental disabilities and IDD
Minimum of 2 years' experience with DBHDD policies, HRST processes
Education
Required Minimum Education:
Associates Nursing or Diploma (Nurse) Nursing; or
B.S. in Nursing (preferred).
Required Minimum License(s) and Certification(s):
All certifications are required upon hire unless otherwise stated.
BLS - Basic Life Support or ARC-BLS - Amer Red Cross Basic Life Support or BLS-I - Basic Life Support Instructor
RN - Reg Nurse (Single State) or RN-COMPACT - RN - Multi-state Compact
