Part-time:16 hours per week and occasional other hours providing on-site direct care/supervision plus one weekend per month on-call.


Qualifications
Qualifications:    
    Current MN RN license
    3-5 years experience working with residents with memory impairment
    Experience working with MDH Comprehensive Home Care license in a residential care setting and/or home care preferred
    Excellent assessment skills
    Ability to transfer knowledge to Home Health Aides/CNAs
    Strong team building and mentoring skills
    Strong organizational and analytical skills
    Ability to make independent decisions
    Experience with aging in place philosophy of care
    Experience with end-of-life process preferred
    Demonstrates ability to communicate effectively verbally and in writing

The RN must have a valid, current MN driver's license and have access to a reliable personal car to use for work transportation.
