Flu Vaccine Considerations
Proof of annual flu vaccination is required for all employees.

An Internal Medical Group has an opportunity for Advanced Practice Registered Nurse (APRN) or a Physician Assistant (PA) to work Part Time in Skilled Nursing Facilities in the Newington area. This is an excellent opportunity for those that enjoy a flexible work schedule, working autonomously, and accelerate in a pay for performance environment.

The Nurse Practitioner or Physician Assistant will travel to Skilled Nursing Facilities contracted with PACT, LLC.

Essential Function

  • Provide medical care to a complex patient population in the short term rehab and long term care setting
  • Analyze patient’s medical history, medication allergies, physical condition and examination to determine best care for acute and chronic medical conditions
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient’s clinical problems and health care needs
  • Prescribe pharmaceuticals, other medications, and treatment regimens as appropriate for assessed medical conditions
  • Record in medical record the physical findings, formulated plans, and prognosis based on patient’s condition within appropriate time frame
  • Coordinate with the nursing facility care planning team in the development of comprehensive care plan
  • Communicate with patients and family members regarding admission, care plan, discharge needs, prognosis/end of life care as needed
  • Follows established departmental policies, procedures, and objectives, continuous quality improvement objectives , and safety, environmental, and/or infection control standards

Education and Experience

APRN

  • The Advanced Practice Registered Nurse is masters or doctorate prepared
  • Certified Nationally as a Nurse Practitioner through AANP or ANCC .
  • The Nurse Practitioner must have possession of a valid (Connecticut) license as a Registered Nurse and an Advanced Practice Registered Nurse, and be certified as a y) Nurse Practitioner by an approved certifying body.
  • DEA, and Ct controlled substance license
  • Experience working with elderly patient population
  • Experience working with EHR
