Job Summary
The Registered Nurse I - Residential delivers appropriate and quality health care services to youth in the residential setting.
This is a staff RN position, on a 3:00pm-11:30pm schedule, with flexibility to be 24-32 hours as well as on which days of the week, though note staff are required to be on a weekend rotation of at least 1 weekend day every other weekend. This role will be based on our Monroe Campus in Rochester, NY.

Essential Job Functions
  • Assess clients with illness, injury, or health related/psychological concerns; develop and implement care plans, and maintain medical records.
  • Consult and coordinate with treatment team providers such as the treating physician, nurse practitioner, and clinician, as well as others to assess, plan, implement, and/or evaluate patient care plans while assuring continuity and quality of care.
  • Transcribe all medical orders accurately and efficiently and carry out orders for treatment as prescribed.
  • Report any significant finding to the appropriate medical provider and guardian when appropriate and carry out the prescribed intervention.
  • Support all members of the team in support of the care plan(s); assure quality care is delivered in an effective, timely, and team-based manner.
  • Communicate respectfully, accurately and clearly both in verbal and written form with all team members and families.
  • Administer nursing care for medically prescribed orders for treatment.
  • Administer medication safely, accurately and in accordance with New York State and agency guidelines. Monitor patients for any reactions or adverse side effects and report medication side effects to prescriber.
  • Administer first aid and CPR during emergency situations and coordinate patient care until Emergency Medical Services (EMS) arrives or the youth is transported to hospital emergency department.
  • Triage phone calls regarding health-related concerns of clients.
  • Oversee and supervise care provided by the LPN and other health office staff to ensure proper nursing care.
  • Provide assessments, monitoring and care to the youth in the event that a physical restraint is needed, and ensure all regulatory requirements for managing and documenting restraint are followed.
  • Maintain timely, accurate, and detailed documentation and progress notes in the patient medical records.
  • Independently complete commitments and take responsibility for actions; recognize and fix errors and omissions.
  • Advocate on behalf of youth to establish appropriate health care.
  • Consult with pediatrician or other members of the health care team to follow up on medical concerns, including physical and psychiatric concerns. Implement follow up care recommendations.
  • Identify needs and provide education to staff and youth as needed on topics relevant to child and adolescent health, such as health education, disease prevention, and health improvement programs. Monitor to ensure proper care and follow up needs are addressed.
  • Understand and adhere to all federal, state, and local statutes, regulatory agency standards and Hillside policies including but not limited to all federal, state, and local statutes, regulatory agency standards, and Hillside policies, including but not limited to, Office of Mental Health (OMH), Office of Child and Family Services (OCFS), Council on Accreditation (COA) and New York State Education Department (NYSED).
  • While this job description covers many aspects of the role, employees may be required to perform other duties as assigned.

Education & Experience
  • Successful completion of an accredited Associate or Bachelor's Degree Nursing Program required.
  • Minimum of 1 year related work experience required.

SPECIAL REQUIREMENTS
  • Active, unrestricted RN license in NYS required.
  • Unrestricted, valid NYS driver's license for minimum of 1 year with a clean driving record and minimum insurance coverage that meets agency standards.
  • Basic Life Support (BLS) training within 60 days of hire and recertification annually.

  • Ability to recognize signs of current and emerging health needs of children/youth and address issues proactively and effectively communicate with children/youth and families to assure appropriate understanding of treatment, issues, and objectives.
  • Possess and maintain accurate knowledge of all regulatory and practice regulations and treatment protocols with special attention to HIPAA requirements.
  • Demonstrate the highest standards for ethical and professional conduct at all times and towards all youth and families.
  • Proficiency with computer-based applications in order to independently complete work and add/use data.
  • Demonstrate a thorough knowledge of services provided both by the Agency and by external service providers and maintain contacts as needed in order to efficiently access care for clients.
  • Demonstrate a commitment to quality using resources efficiently and with the greatest effectiveness.

    • Weekly lifting up to 50 lbs. (community-based staff may be required to lift an empty wheelchair weighing up to 50 lbs.), 10-15 lbs. on a daily basis
    • Weekly carrying up to 55 lbs. approximately 10-100 ft., 10-15 lbs. on a daily basis
  • Manual dexterity is required, including the frequent ability to grasp in both hands and continuous use of fine manipulation skills in both hands (approximately 1-2.5 hrs.)
  • Occasional exposure to dust, fumes, gases or chemicals is apparent.
  • Protective clothing or equipment as required: Close toed shoes; Protective Shields, Universal Precaution PPE, Gloves for toileting and food preparation
  • Ability to change positions as needed

$33.00 Minimum pay rate, $43.50 Maximum pay rate, based on experience.
*These pay rates may include a Regional and/or Department premium.
