Job Summary

Responsible for coordinating the day-to-day functions for assigned area. Assists with ensuring that departmental goals are achieved, timelines are met and service excellence is provided in all aspects of work. Subject matter expert. Responsible for the provision of direct social work service in Home Health and/or Hospice utilizing the professional healthcare process of assessment, problem identification/planning, action/intervention and clinical goal/outcome evaluation. In addition, provides leadership in promoting and integrating the Social Work Code of Ethics and the mission, values and vision of PeaceHealth into the culture and operations of the organization.

Details of the position:

  • Assigns work; plans and manages priorities; provides technical assistance; oversees staff schedules; monitors quality of work; monitors team projects; mentors staff; provides constructive feedback; oversees staff training; ensures quality improvement; provides leadership feedback on staff performance; and assists with goal setting for the team.
  • Providing direct casework services to patients and families related to their medical or psychological diagnosis, including biopsychosocial assessment, assisting with long-term planning and decision making, resources support, crisis intervention and short-term therapeutic interventions.
  • Performing holistic evaluation of the patient in their chosen care setting, developing an appropriate care plan that is rooted in patient and family goals, assisting with transfers, placement and discharge planning, initiating discharge planning, evaluating progress towards goals and monitoring achievement of desired outcomes.
  • Providing skilled social work services to patients and families, including emotional support, coordination of care, therapeutic interventions, and education pertaining to hospice philosophy, goals of care, and hospice plan of care.
  • Performing on-going holistic assessment and modifying the care plan with the patient, caregiver, case manager, primary physician and other members of the interdisciplinary interdisciplinary care team, as clinically indicated.
  • Evaluating patient/home environment as related to holistic healthcare/social needs and safety, and making appropriate referrals to other services, agencies and community resources as clinically indicated in collaboration with the case manager and primary physician.
  • Coordinating social work services, collaborating and communicating with other members of the interdisciplinary team to ensure an appropriate and smooth flow of care/services for the patient through the care continuum.
  • Evaluating, reviewing, and revising the patient’s social work plan of care on a regular basis and, as clinically indicated, taking prompt action in response to changes in the patient’s health condition in collaboration with the case manager.
  • As part of on-going hospice recertification, regularly assessing patient’s disease progression and completing timely clinical documentation of decline and providing assessment during interdisciplinary team meetings.
  • Providing services to a generally adult patient/caregiver population , with comfort level/interest in working with cognitively impaired and medically fragile patients. May also provide services to pediatric population.
  • Providing services to all patients utilizing appropriate universal infection control precautions and practices (standard and transmission based precautions), including individuals with communicable diseases.
  • Provide skilled and timely clinical documentation into the hospice electric medical record (EMR), in compliance with regulatory, quality and Agency guidelines.
  • Performing quality/outcome oriented home visits and other job duties according to Agency productivity standards.
  • Proactively initiating and participating in patient care interdisciplinary Care Conferences. Participate in Interdisciplinary Team meetings, staff meetings, as well as education and in-service programs.
  • Assist in development and facilitation of Group Supervision, support groups and Social Work team meetings and support.
  • Performs other duties as assigned.

What you bring:

  • Master's Degree Required: Social Work from an accredited school of Social Work
  • Minimum of 2 years Required: experience as a Medical Social Worker 
  • Preferred: Previous leadership experience 
  • Preferred: Previous group facilitation experience 
  • Required: Upon Hire Licensed Independent Clinical Social Worker-Washington 
  • Required: Upon Hire Basic Life Support 
  • Required: Upon Hire Drivers License and auto insurance
