Job description

WORK SCHEDULE: Six hours per week: one day six hours OR two days, three hours each day
HOURLY RATE: $17.00 per hour
Position Overview:
The Peer Educator is to provide outreach and supportive service to Women of Color. The Peer Educator will assist the WOC team members and provide support and services to identify newly diagnoses, women who are lost to care, identify the Social Determinates of Health for Lack of Nutrition services. This work is carried out in support of the mission and goals of the Ryan Health Network.
Essential Functions:
  • Orient new patients to the health centers' HIV services and staff
  • Conduct re-engagement activities and outreach to patients
  • Provide support, education, and adherence assistance
  • Educate patients regarding early medical intervention, options to reduce/modify stressors, and adherence to treatment regimens and appointment schedules
  • Provide reminder calls and follow-up for missed appointments and home visits as needed
  • Conduct group and individual level educational and behavioral interventions utilizing evidence-based curricula and education materials as needed
  • Conduct home visit and escort to medical/social service appointments
  • Ensure that programmatic records are maintained according to agency standards
  • Documents all outreach efforts, health education and group interactions in eCW

  • Immediately report any problems or unusual occurrences to supervisor.
  • Other special projects/assignments as needed.

Minimum Experience and Skills Required:
  • Certified in Peer Education Training
  • NYS adult resident, aged 18 and up
  • Experience working with the target population
  • Ability to relate well to target population
  • Experience working with other Peers.
  • Must demonstrate a willingness to learn
  • Must demonstrate knowledge and skills necessary and willingness to go into the field to make home visits
  • Must have computer skills in order to document in the electronic medical record system (eCW)

Education, Licenses and/or Certifications Required:
High School Diploma or GED
Preferred Qualifications:
Bilingual English/Spanish preferred
