Job Summary:
A Registered Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and the plan of care, under the direction and supervision of the Director of Clinical Services/Clinical Manager.

Qualifications:

  1. Graduate of an approved school of professional nursing and currently licensed in the state(s) in which practicing.
  2. Two (2) years of nursing experience preferred, with at least one (1) year of experience in Home Health preferred but not required.
  3. Acceptance of the philosophy and goals of this Agency.
  4. Ability to exercise initiative and independent judgment.
  5. Valid RN or LPN license in the state of Nevada.
  6. CPR certification required; ACLS preferred.
  7. Proficiency in electronic health records (EHR) and familiarity with healthcare documentation requirements.

Key Responsibilities:

  1. Patient Assessment and Care Planning

    • Conduct initial and ongoing assessments of patients' physical and mental health.
    • Develop individualized care plans based on patient assessments, medical history, and physician orders.
    • Regularly update care plans to reflect any changes in patients' condition or needs.
  2. Direct Patient Care

    • Administer prescribed medications, monitor vital signs, and provide wound care.
    • Support patients in daily activities and self-care, providing education on condition management.
    • Ensure adherence to infection control protocols and safety procedures.
  3. Coordination of Care

    • Collaborate with physicians, therapists, social workers, and other members of the healthcare team to ensure comprehensive care.
    • Communicate any changes in patient condition promptly to the supervising physician and healthcare team.

    • Educate patients and family members on managing their health condition, medication administration, and the use of medical equipment.
    • Provide guidance on lifestyle adjustments, nutrition, and preventive care to improve health outcomes.
  5. Documentation and Compliance

    • Maintain accurate and timely patient records in accordance with ACHC standards, state regulations, and agency policies.
