Job description

Location: Primary location will be HF Ascension Novi but must be able and willing to cover shifts if needed at all HF Ascension Hospitals.

Registered Nurse: Hospice Acute Care Coor

Shift: Part time; every other weekend rotation along with holiday rotation.

GENERAL SUMMARY:

Functions as a care coordinator for Hospice within the hospice setting. Assesses and identifies patient needs in accordance with the hospice concept of care. Coordinates with interdisciplinary team to provide complete patient/family care and referral. Provides pain management and symptom control. Maintains the continuity of care by ensuring patient needs are met.

Unit Description: Qualified candidate will have RN degree, one year of Med/Surg experience, Hospice experience is preferred. Candidate should be able to work independently but also cohesively with team members. Qualified candidate should also be able to build strong relationships with key stakeholders including but not limited to, hospital Case Managers, providers, and unit staff. Our department will work as Hospice Liaisons within the hospital setting, including but not limited to, evaluating and assessing patients for hospice inpatient and also assisting with complex hospice consults/referrals.

 EDUCATION/EXPERIENCE REQUIRED: 

  • Requires an Associate's Degree or Diploma in Nursing. 
  • Requires understanding and acceptance of the Hospice concept of care, and a working knowledge of current pain and symptom management.
  • Requires interpersonal skills to present a positive image of hospice to those whom are referred to the program. 
  • Possess the ability to be flexible in adapting policies and procedures to meet the needs presented by hospice patients and families. 
  • Previous hospice experience preferred. 

CERTIFICATIONS/LICENSURES REQUIRED: 

  • Requires current licensure in the State of Michigan as a Registered Nurse. Hospice certification preferred.
