Job description

Position Summary: The Community Habilitation Worker assists individuals with disabilities in achieving greater independence and self-fulfillment. Responsibilities include implementing individual's community habilitation plans, assisting and training in daily living skills such as: cooking, cleaning, shopping, and assisting program participants in developing a strong personal network within their community. Comply with all agency standards, rules, practices, policies, procedures, and medication administration. The position is part-time. Work schedule includes evenings and weekends.

Qualifications: The ideal candidate must possess strong communication skills. The ability to work independently in a team environment is necessary, as is flexibility to meet program needs. Knowledge of and respect for diverse populations is required. High School Diploma or GED with one year of paid employment, community service and/or training is a plus. Human service experience is desired.

Other Requirements: Must have a valid driver's license, a good driving record, the use of vehicle, and the willingness to transport program participants in a personal and/or agency vehicle. Desire to do meaningful work and make a difference in someone's life is a must.
