Job description

Memory Health Program Coordinator
Schedule: Part-Time 16 Hours | Tuesday amp; Thursday
Position Summary
The Memory Health Coordinator supports and coordinates memory care services for Residents with cognitive changes, promoting their physical, social, and emotional well-being. This role assists in delivering memory-related programs, provides guidance to team members, and collaborates with the Manager of Memory Health Services to ensure best practices in dementia care across the continuum.

  • Delivers person-centered, evidence-based programming that supports cognitive and emotional well-being
  • Provides and receives constructive feedback to continuously improve services
  • Advocates for Residents and communicates effectively with families, Team Members, and other departments
  • Adapts programming to meet Resident needs and provides Team Members with tools and strategies for effective memory care
  • Monitors and reports on changes in Resident behavior or condition to appropriate Team Members
  • Guides Team Members in promoting positive outcomes for Residents
  • Collaborates with the Memory Health Manager and other departments to ensure coordinated care
  • Builds trusting relationships with Residents and families to support quality of life
  • Assists in implementing, maintaining, and evaluating memory care programs
  • Prepares reports and documentation to inform decision-making and support team communication
  • Identifies program needs and contributes to ongoing improvements
  • Creates a welcoming, personalized experience for each Resident
  • Promotes engagement by tailoring activities and interactions with individual interests and abilities
  • Meets department uniform, appearance, and grooming requirements
  • Complies with all OSHA safety regulations, applicable local laws regarding health, safety or environment
Required Skills and Abilities
  • Effective communicator in one-on-one and small group settings
  • Strong organizational, time management, and record-keeping skills
  • Demonstrates sound judgment and decision-making in routine situations
  • Work professionally with people from a wide variety of cultural and socio-economic backgrounds
  • Must possess the ability to deal tactfully with Team Members, Residents, family members, visitors, and the general public
  • When working in Resident areas, must be able to communicate effectively in English
  • Complies with protection of health information (HIPAA) safeguards
  • Attends and participates in in-service educational classes, Healthcare Academy and staff meetings as directed
Education and Experience
  • High school diploma or equivalent required
  • Experience in senior living and/or dementia care preferred
  • Willingness to complete dementia-related training
  • Uses basic Microsoft programs (Word, Excel), email, and office equipment
  • Basic knowledge of dementia and cognitive decline
Satisfactory completion of a background investigation is required for this position.

Pay Range: $25/hour
