Schedule is 20 hours: Mon 9a-1p, Tues-Thurs 1p-5p, Fri 9a-1p

Hourly rate starts at $23.00

ESSENTIAL DUTIES AND RESPONSIBILITIES: To perform this job

Successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions to include the following:

1. Maintain consumers records in accordance with LifeStream policy.

2. Maintain updated Health Care Records for all consumers related to medication changes, medical appointments, diagnosis and medical history.

3. Assists Manager with completing ISP documents in HCSIS.

4. Assist Manager with conducting monthly home visits.

5. Review daily attendance and individual shift logs submitted by Personal Advocates for accuracy, content, etc

6. Assists Manager with chart reviews.

7. Maintain updated record keeping in ehana. Review HCP’s for accuracy upload into consumers file. Update Emergency Fact sheets and clinical database as pertinent information changes.

8. Ensure consumers training occurs according to established guidelines and appropriate documentation is maintained.

9. Assist with facilitating pre – isp meetings. Maintain contact with program Manager to ensure appropriate implementation and progress on consumer ISP goals and objectives.

10. Act as a member of the team offering input into all programmatic services.

11. Make recommendations regarding program quality, consumer choices and provide general input to the program Manager as needed.

12. Makes recommendations to the program Manager relevant to staff training needs.

13. Demonstrates flexibility in meeting program needs.

14. Assists with Quality Assurance Checks, at least quarterly, on assigned program records.

15. Assist with uploading files and programmatic systems for new program participants.

16. Assist Manager to ensure financial record keeping is accurate and submit to the Financial Assistant monthly.

QUALIFICATIONS AND REQUIREMENTS:

· Maintenance of an acceptable CORI

· Valid Driver’s license

· Negative drug screen

· Use of personal vehicle for work purposes

· Commitment to the philosophy of community inclusion

· Maintain flexibility with weekly schedule to accommodate program needs.

EDUCATION and/or EXPERIENCE:

· Demonstrated history of quality work performance and teamwork in previous positions

· Proven Leadership skills
