BSON-Behavioral Support Ontario Nurse

Health Services, Long Term Care

Status: Regular Part Time

Salary Range: $39.25 -$42.22 per hour (as per collective agreement)

Work Mode: Onsite 

Location: Sheridan Villa LTC, 2460 Truscott Dr, Mississauga ON L5J 3Z8 

Hours of work:10am - 6pm, 64 hours (8 shifts) bi-weekly including alternate weekends, flexible hours to meet the needs of the role

POSITION SUMMARY

Reporting to Director of Care (or designate) you will be responsible for the provision of quality, resident-centered care based on the College of Nurses of Ontario and the Ministry of Health and Long-Term Care legislated requirements, the Regional Values, policies and procedures, and best practice guidelines. 

The Behavioural Supports Ontario Nurse is a member of the interdisciplinary team and works within the Behavioural Supports Ontario framework to support persons living with responsive behaviours associated with dementia, mental health or other complex neurological conditions.

MAJOR RESPONSIBILITES

• Performs all duties under the direction of the Director of Care (DOC) and/or Designate, and in accordance with the Code of Conduct, and the Regional Values of Empowering, leadership, purposeful change, transparency, Supportive care, togetherness and Customer Service. 
• Provides nursing care in accordance with the College of Nurses of Ontario' Standards of Practice, administrative policies and procedures of the centre, and other applicable legislation.
• Applies the Nursing Process within the behavioural assessment of each resident/client, and the development, implementation and evaluation of plan of care.
• Records medications and treatments as prescribed by the physician/Neuro-Behavioural Nurse Practitioners in collaboration with RPN/RN, and in accordance with the College of Nurses of Ontario's Standards of Practice as applicable.
• Observes, assesses, evaluates, records, and reports behavioural outcomes to physician/ Neuro-Behavioural Team, DOC and/or Designate.
• Supports the completion of risk management reports in consultation with RN/RPN and completes analysis trends as it relates to incidents as applicable.
• Completes residents' incident reports, identifies, analyzes and implements corrective actions in consultation with the RN/RPN and interdisciplinary team as applicable.
• Provides behavioural support, guidance, and leadership to the interdisciplinary team. 
• Provides knowledge transfer of behaviour management strategies to build capacity of team members through one-to-one group sessions, role modeling, Prevention and Management of Responsive Behaviour Program, and behavioral huddles. 
• Assists with the admission process of residents identified with responsive behaviours.
• Provides leadership in implementing the Responsive Behaviour Program.
• Completes applicable behavioural assessments, evaluation, and collaborates with the interdisciplinary team to develop, communicate and implement strategies for behavioural management.   
• Collaborates, supports and provides information to residents/clients and families regarding responsive behaviours and plan of care.
• Completes responsive behavioural referrals received from the interdisciplinary team and provides recommendations in a timely manner.
• Facilitates referrals to external resources and implements plan of care based on external consultant recommendations in collaboration with the interdisciplinary team.
• Promotes resident/client centered approach, and more consistency of care.
• Provides point of care education to provide effective interventions to residents/clients.
• Collaborates with the Neuro-Behavioural Team/external Specialists, completes the required behavioural assessments and monitors the effectiveness of pharmacological and non-pharmacological interventions. 
• Facilitates the Responsive Behaviour Management Committee meeting quarterly, or as applicable.
• Participates in quality improvement initiatives to support the ongoing Prevention and Management of Responsive Behaviour Program development and evaluation, including the completion and submission of reports to the LHIN(s).  
• Reports health and safety hazards, actively participates in eliminating causes of accidents, and participates in action plan development as required. 
• Respects and promotes comfort, safety and dignity of all clients/residents.
• Performs other delegated duties as assigned.
• Collaborates with external stakeholders on admission and re-admission as required.

EDUCATION, EXPERIENCE AND QUALIFICATIONS

• Current certificate of competence as a Registered Practical Nurse (RPN) with the College of Nurses of Ontario.
• If graduation was prior to 1985 must have successfully completed an approved medication administration course for the R.P.N.
• One to two years’ experience working in geriatric care or Long-term care.
• Demonstrates understanding of the normal aging processes.
• Knowledge of dementia and the ability to assess the unique impact of the disease.
• Demonstrates knowledge of dementia, delirium, complex mental health conditions and effective care strategies as it relates to:

1. 1. Most prevalent types and related causes
   2. Disease processes, stages and progression
   3. Diagnostic and assessment processes
   4. Cognitive or neurological symptoms
   5. Current treatment interventions both pharmacological and non pharmacological
   6. Understanding the concept of responsive behaviours and behavioural expressions as a form of communication
   7. Promoting optimal quality of life
   8. Understanding the disease process from the perspective of the resident/client and family members.

• Understanding of pharmacological agents, interactions and adverse effects in managing behavioural and psychological symptoms of dementia (BPSD)
• Must have excellent critical thinking skills for early detection, intervention and prevention of responsive behaviours.
• Knowledge of problem-solving approaches related to behavioural de-escalation.
• Demonstrates knowledge and skill working with older adults presenting with or at risk for responsive behaviours that may be associated with substance abuse and other neurological conditions is preferred.
• Knowledge of GPA, PIECES™, U-First, Montessori, Dementiability.
• Current CPR and First Aid Certification.
• Must have effective oral and written communication skills in English, a good understanding of resident/client-centered care.
• Must be able to communicate appropriately with residents/clients, staff, visitors, volunteers and medical professionals.
• Ability to approach highly sensitive issues with professionalism.
• Proficiency in basic computer skills including the ability to create reports is a requirement.
• Must be physically able to perform all delegated duties.
• Flexible work hours and ability to cover all shifts. Hours may be determined by Director of Care or designate as per operational needs.
