Your Role:
The Physical Therapist is responsible for evaluating, planning, and carrying out individualized treatment programs for home health patients while coordinating care with the broader rehabilitation and clinical team.

Key Responsibilities:
โ€ข Provide effective patient care using approved therapy techniques and procedures
โ€ข Evaluate patient condition and establish measurable treatment goals
โ€ข Develop and implement individualized treatment plans
โ€ข Supervise and coordinate care with PTAs and HHAs
โ€ข Document patient progress, updates, and treatment notes accurately and on time
โ€ข Educate patients and families on exercises, mobility, and use of adaptive equipment
โ€ข Evaluate home environments and recommend modifications to improve safety and independence
โ€ข Ensure all services meet HIPAA standards for confidentiality
โ€ข Coordinate with nursing, social work, and other clinical staff to ensure comprehensive care

Qualifications:
โ€ข Active Physical Therapist (PT) license in FL
โ€ข Minimum 1 year of PT experience preferred
โ€ข Physical exam within the last 12 months
โ€ข Current TB test (within 12 months) and CPR certification
โ€ข Valid driverโ€™s license and reliable transportation
