Job description

JOB TITLE: Home Health RN Case Manager & Recertification Nurse (RNCM)

This is a Part-Time or PRN leadership position with our agency, with two employment options:

  1. PRN for local / 30-60min travel, or
  2. PRN or Part-Time for 2-hour travel radius (and first priority of cases to secure additional hours)

HIGHLIGHTS & REQUIREMENTS:

  • This role works closely with and reports directly to our NW OH Regional Program Administrator. 
  • For those pursuing career growth: a defined path for advancement to Assistant Program Administrator, Program Administrator, Clinical Administrator, Director of Nursing (DON or Asst. DON), Business Administrator, QA / Compliance leadership, and other positions will be supported.
  • Work consists of: case management, starts of care (SOC), admissions, recertifications, transitional care and resumptions of care (ROC), clinical reviews, and other assessments and services, as well as new hire orientations, in-service and supervisory reviews, and input to influence our training and education, care practices and outcomes, and the success of our clients, employees and business overall.
    • Requires back-up and occasional Skilled Nursing visits and services including med sets, dressing changes, etc.
  • At least 1 year of experience is required in: Case Management, Case Admissions and Discharges, wound care, PICC/PORT/IV Care, Transitional Care, lab draws, OASIS and Non-OASIS assessments, and excellent documentation and field staff leadership.
  • At least 1 year of paid experience is required at/with a home health agency or in-home care provider (or equivalent); 2-5 years' experience is preferred.
  • Desirable but not required:
    • Nurse Supervision experience
    • People management
    • Peer/staff education and development
    • Success or willingness to build relationships with client payors, case mgr / entity relationships (i.e., AAA, DoDD County Board, VA, etc.) in NW Ohio and our current + prospective employees in these communities.

SCHEDULE TYPE & SERVICE AREA: 

This is a weekday position (Monday - Friday) requiring primarily daytime availability (8am -5pm) with occasional evening availability. No Weekends required.

  • Option 1 / Regional RNCM (PRN Only): 
    • Requires travel as needed to all clients throughout Findlay, Fostoria, Tiffin, and surrounding areas within 30-60 minutes travel.
    • Requires at least 70% case acceptance rate within your agreed-upon availability and at least 1 case worked per 15 days (with exceptions) - though more is desirable if needed.
  • Option 2 / Road Warrior RNCM (PRN or Part-Time):
    • This service area spans the entire NorthWest Ohio region, including Toledo, Holland, Perrysburg, Findlay, Richland, Marion, Mansfield, Fostoria, Tiffin, Norwalk, and all surrounding areas within a 2-hour radius.
    • Willingness to travel throughout this entire region is required.
    • This role will receive first priority of cases (over PRN-Only RNs) to maximize additional hours with our agency.
    • Requires at least 50% case acceptance rate within your agreed-upon availability and at least 1 case worked per 15 days (with exceptions) - though more is desirable if needed.
    • While Findlay/Fostoria/Tiffin are a centralized "home base" to cover this region with minimal travel, Qualified Applicants may reside in or near any part of the service areas so long as travel requirements are fulfilled.

EMPLOYMENT TYPE: Long-term employment for Part-time or PRN RNCM/Recert Nurses*

  • No On-Call requirements / No Overnight travel required.
  • Weekend services are optional when available but are not required.

Job Summary
As an RN Recertification Nurse, you will be instrumental in coordinating, re-evaluating and confirming the medical necessity of continued home health care services for our patients, and providing patient care and ensuring optimal outcomes.

  • With core skills in managed care, patient assessments and compliance, client care and treatment plan documentation, and discharge planning, you will oversee patient services and provide expert nursing care.
  • You will submit updated documentation of the patient's condition and ongoing care needs timely and within compliance of all state, federal, and agency guidelines.
  • You will ensure the successful communication with and between our physicians, our agency staff, service administrators, other caregivers and clinicians, and the client + his/her family.

Responsibilities/Requirements of an RN Case Manager / Clinical Supervisor include:

  • RN performs client assessments while directing, planning, and initiating appropriate actions in home care situations.
  • RN manages/supervises a team of RN, LPN, Home Health Aide (HHA), and Personal Care Aides.
  • RN to Document/Communicate/Coordinate patient status/change in condition to care team.
  • RN to perform job duties while promoting personal safety and a safe environment for clients and coworkers.
  • Willing to travel surrounding counties.
  • Minimum of one (1) years of nursing supervisory experience. Home care experience preferred.

Reports to: Regional Program Administrator, or Clinical Administrator/Director of Nursing


Qualifications

  • Graduate of an accredited School of Nursing.
  • Minimum of 1 year experience as a Registered Nurse, with an active RN License / ability to practice in the State of Ohio.
  • Minimum of 1 year of experience in home health agency or equivalent setting.
  • CPR and First Aide Certifications required (or can be obtained prior to hire date through our agency or accredited third parties).
  • Experience with OASIS and NON-OASIS assessments is also required.
  • Strong background in nursing, patient care, and confidentiality.
  • Exceptional on-time and attendance record, and consistent availability aligned to your chosen schedule type (PT/FT/Hybrid)
  • Desirable character traits: reliable, honest beyond reproach, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful, and positive.
  • Familiarity with HIPAA regulations and medical terminology.
  • Willingness and ability to perform required physical demands (see below)
  • Consent + Ability to pass a State & Federal background check (including fingerprints)
  • 18yo + active Driver's License / State ID + US Work Authorization
  • Reliable transportation to all scheduled visits, within full reach of stated travel radius and assigned clients (up to at least 2 hours is required).
