  • Provide inpatient palliative care consultations and follow-up care for patients with serious illness across the AHN Palliative Care Program.

  • Partner with physicians for collaboration while functioningat the top oftheAPP scope of practice.

  • Support patients and families through symptom management, hospice evaluations, goals-of-care conversations,and end-of-life planning.

  • Participate in interdisciplinary collaboration with physicians, APPs, hospice liaisonsupportand palliative counselors.

  • Procedures are notrequiredfor this position.

  • 2 to 3 years of Hospice and/or Palliative Care experience, or related experience caring for patients with serious illness.

  • Hospice and Palliative Medicine (HPM) certification preferred.

  • Strong skills in symptom management, serious illnesscommunicationand end-of-life care.
